Name Change
Name Change Request:This will require a copy of a legal document reflecting the exact way your name should be displayed. You may submit the change request through the new NBCRNA portal login:
• Login
• Select Name Change
• Select Create New Application
• Review instructions
• Follow the prompts to compete and submit your request.
• Please do not send your social security number or a copy of the card
After the NBCRNA has processed your request, you may print a copy of your updated card via the NBCRNA portal. Please follow these steps:
• Login
• Select My Account
• Select Wallet Card
• Upload
